What went wrong

Claude Code, Opus. A repo set up with this skill still grew a 6,000-line README that restates the code, because specs and tickets kept asking for "the README describes X". domain.md says how to read GLOSSARY.md and ADRs, not where new knowledge goes.

Proposed change

A short "Where knowledge goes" section in domain.md, and one line in the ## Agent skills block: code, tests and config are the reference; terms go in GLOSSARY.md, hard-to-reverse decisions in ADRs, a tool's how-to in its --help; no doc, spec or ticket restates the code.
